Working With Concurrent Disorders

Expansion of the Concurrent Disorders Transitional Support Partnership (CDTSP) is shaped by the partnership’s funding and planning requirements as well as an interested agency’s:

  • Current operation in the Toronto Central LHIN (downtown Toronto).
  • Its capacity for serving persons who are marginalized and have a concurrent disorder.
  • Its ability to meet the service needs of the partnership and to serve these clients on a prioritized basis.

To consider joining, an agency should understand the process of becoming a partner, that is:

  • Determine whether the partnership is currently considering expansion.
  • Write a letter or email to the manager of Concurrent Disorders Support Services (CDSS), indicating interest and describing the client population, relevant services, need for the partner services and potential offerings.
  • Meet with representatives of CDSS to discuss CDSS operations, the agency’s offerings and any mutual benefits of collaboration.
  • Await approval by the partnership.
  • Collaborate to draft a partnership agreement.
  • Attend a management orientation and schedule staff presentation(s) of CDSS and its operations.
  • Start attending partnership meetings.
